How to make a referral
Step 1
Reach out
Call or message the RHI team for Boise/Treasure Valley or Idaho Falls. Families, guardians, and case managers can all start the conversation.
Step 2
Talk with us
We learn about the person's needs, goals, and current supports, and answer questions about our services.
Step 3
Coordinate
We work with the person's case manager or support coordinator on eligibility and service authorization through Idaho's programs.
Step 4
Plan and begin
Together we build a person-centered plan and schedule the start of services.
